Complete Pelvic Floor Physiotherapy will not disclose any information that Complete Pelvic Floor Physiotherapy gather about our staff or clients to any third party except as required by law. Details collected are: name, address, phone numbers, date of birth and information relevant to your condition. This information is collected when clients fill in the client intake form or provide information during the consultation with the therapist. This information is kept electronically on Complete Pelvic Floor Physiotherapy’s computer system or kept as a hard copy at the premises of Complete Pelvic Floor Physiotherapy. Complete Pelvic Floor Physiotherapy may not be able to provide services without your personal information.

Complete Pelvic Floor Physiotherapy uses the information collected only for the services provided by Complete Pelvic Floor Physiotherapy. The client can access this information by phoning or visiting the office to discuss the request with the reception staff.

No staff member or client information is shared or sold to another organization. If staff member or client information is required by a third party, except as required by law, Complete Pelvic Floor Physiotherapy will obtain the written consent of the relevant staff member or client prior to release of any information. If Complete Pelvic Floor Physiotherapy disclose to overseas parties, it will be sent via a secure passage.
